NCLEX-RN Blueprint and Client Needs Categories
The NCLEX-RN is built around the nursing decisions a newly licensed registered nurse must be able to make safely. The 2026 test plan organizes exam content by Client Needs, with clinical judgment built throughout the examination. This guide breaks down the current blueprint so you can see where the exam places its emphasis and how those areas connect to your preparation.
How the NCLEX-RN test plan is organized
NCSBN organizes the test plan into four major Client Needs categories. Two of those categories are divided into subcategories, creating eight content areas that are commonly shown when discussing the NCLEX-RN blueprint.

Three six-item case-study sets explicitly measure clinical judgment.
Approximately 10% of items also measure clinical judgment outside case studies.
The eight NCLEX-RN Client Needs content areas
The percentages below are the official 2026 test-plan ranges. The single percentage shown as the target is the midpoint used by MyScoreMetrics when building balanced practice content and performance tracking.
Management of Care
15–21%Focuses on providing and directing nursing care that protects clients and health care personnel. Questions may involve prioritization, delegation, assignment, advocacy, ethics, client rights, continuity of care, confidentiality and collaboration with the health care team.
Safety and Infection Prevention and Control
10–16%Covers nursing actions that protect clients, staff and others from health and environmental hazards. Expect concepts related to infection prevention, standard and transmission-based precautions, safe equipment use, injury prevention, emergency response and error prevention.
Health Promotion and Maintenance
6–12%Measures the nurse's ability to support health across the lifespan. Content can include growth and development, pregnancy, newborn care, screening, prevention, health teaching, lifestyle choices and early recognition of health risks.
Psychosocial Integrity
6–12%Focuses on emotional, mental and social well-being. Questions may address therapeutic communication, coping, crisis intervention, abuse and neglect, grief, substance use, mental health conditions and safe care for clients experiencing behavioral health concerns.
Basic Care and Comfort
6–12%Covers assistance with activities that support comfort, mobility and daily functioning. Topics may include nutrition, elimination, mobility, hygiene, rest, nonpharmacologic comfort measures and the use of assistive devices.
Pharmacological and Parenteral Therapies
13–19%Tests safe medication and parenteral therapy. Content can involve medication administration, adverse effects, contraindications, dosage calculations, intravenous therapy, blood products, central venous access and evaluation of a client's response to treatment.
Reduction of Risk Potential
9–15%Focuses on reducing the chance that a client will develop complications related to an existing condition, treatment or procedure. Questions often require interpretation of findings, monitoring for complications and recognizing when follow-up is needed.
Physiological Adaptation
11–17%Covers care for clients with acute, chronic or life-threatening health conditions. Questions may involve changes in body systems, emergency care, hemodynamic instability, fluid and electrolyte imbalance, medical emergencies and unexpected responses to illness or treatment.
Why ranges instead of fixed percentages? The NCLEX-RN uses computerized adaptive testing and a variable exam length. Because each examination is individualized, NCSBN notes that the content distribution on an individual exam may differ by up to ±3% in each Client Needs category.
Four major Client Needs categories
Although students often talk about eight NCLEX categories, the official structure starts with four major Client Needs categories. Safe and Effective Care Environment and Physiological Integrity are then divided into subcategories.
Safe and Effective Care Environment
Includes Management of Care and Safety and Infection Prevention and Control.
Health Promotion and Maintenance
Covers nursing care that supports normal growth, development, prevention and early recognition of health concerns.
Psychosocial Integrity
Addresses emotional, behavioral, psychological and social needs that affect health and nursing care.
Physiological Integrity
Includes Basic Care and Comfort, Pharmacological and Parenteral Therapies, Reduction of Risk Potential and Physiological Adaptation.
Clinical judgment is part of the blueprint
The NCLEX-RN does not test nursing knowledge in isolation. Candidates must use information from a client situation to decide what matters, what it means and what nursing action should come next.
Six clinical judgment functions
A six-item case study follows one client presentation as additional information becomes available. The questions measure the decisions a nurse makes while interpreting the situation and evaluating care.
Processes integrated throughout NCLEX-RN content
Some nursing skills are not limited to one Client Needs category. The test plan identifies several processes that are integrated throughout the exam.
Use the blueprint to understand where your performance needs work
MyScoreMetrics organizes practice and performance reporting around the NCLEX-RN Client Needs structure. Instead of only seeing a total score, you can use your results to identify which areas need more attention and build your study time around those gaps.
